Do I need to make a report in the accident book to claim compensation for work related injuries?

Hi, I had an accident at work at my previous place of employment.

I slipped on some slippery steps that lead to the bins in the back, the steps had been used to have the mop fluid thrown down them from the kitchen which contained grease from the kitchen floor.


I have really damaged my back, still waiting on the exact results but I was unable to work for roughly a week and a half, given various medication by the doctors and A&E

I was unable to record the incident in the accident book as I had to have a family member take me straight to accident and emergence because the ambulance took too long, I am hoping the first aider on duty took care of this.

should I have had to fill these forms in? because I will not be able to do this now, I left around a month ago and i do not think they have an accident book anyway as it was a bad management team at the time.
